RI Livable Home Modification Grant Application Process
The RI Livable Home Modification Grant reimburses 50% of the cost of modifying homes, up to a total of $5000. Home modification can enable homeowners or renters with disabilities or mobility issues to remain in their own homes safely and avoid relocation to costly long-term care facilities.
Eligibility requirements are very generous; they are spelled out in the attached full grant application package. Essentially, eligibility is based on the income of the individual who has the disability or need, and not on household income.
We created the following easy Guide to simplify the application process. Screenshots of the application are at the bottom of this post for your reference.
SECTION I: Simply complete the patient information.
SECTION II: Complete the property information.
SECTION III – “Needs Assessment” and “Retrofitting of an Existing Unit”—To be completed by a “qualified professional” who is ANY healthcare professional (i.e. Doctor, Occupational Therapist, Physical Therapist, Case Manager).
SECTION III — “Before Photographs” and the required “Estimated Cost” will be provided by Oakley Home Access.
SECTION IV — Applicants who file taxes must complete the tax worksheet and include copies of
W-2s (if applicable). Applicants who do not file taxes automatically qualify.
SECTION V – “Certification” to be signed by Patient.
